I just picked up Innsmouth. It seems like FF got lazy with the directions; typos, wrong monster numbers listed etc…
Anyway, I have 2 questions about it.
Being there are no gate markers, how do monsters spawn on the Innsmouth board?
For the 2 areas not attached by a street (Devil Reef and Y'ha-nthlei). The directions state how you get there, but done mention how you get out. How can you get back to the rest of the board?

Innmouth has unstable locations. Gates can still open there. Just use the AH ones. You need DH or KH if you want variety in the gates tokens.

I'll spoil Tibs post. In order to return from Devil's Reef, you must have a different investigator at Falcon Point use the written ability (pay $2) to bring the investigator back to Innsmouth.
There are 3 ways monsters spawn in Innsmouth.
1) A gate opens and a monster appears, releasing it into the streets. Also, Monster Surges can happen making things even worse.
2) A mythos card directs you to spawn X number of monsters in a certain area.
3) Flying monsters in the "Sky" space on the Arkham Game board can also stalk investigators in street locations in Innsmouth, provided they have the lowest sneak value or are the only ones unfourtunate enough to be in a street. This technically isn't a monster spawn but does provide a way for monsters to enter Innsmouth.
Have fun "rescuing" investigators from Devil's Reef. If you start a game with Silas or Ursula you will find that you can cleverly place them so that they're very usefull in your investigations of Innsmouth.
Full Doom Track
Gates open in Innsmouth just like they do in the base game. If you have a look at the Mythos cards in your Innsmouth expansion you'll see that several of them will cause gates to open at the various unstable locations in Innsmouth. There are, as you say, no new gate markers in the Innsmouth expansion, so you just use whichever gate markers which correspond with your other board(s).
The game does state how you get back as well. The special encounter at Falcon Point states as follows:
"Boat charter: During the Movement Phase, you may spend $2 to move to any aquatic location and have an encounter there (if applicable), or move any other investigator in an aquatic location to Falcon Point. Investigators may not move any further after moving with this location ability."
So if an investigator is at Devil Reef or Y'ha-nthlei, any other investigator can pay $2 at Falcon Point to bring the first investigator back.
In addition to this there are some Innsmouth encounters which allow you to move back to the mainland.
